Persist note and project backlinks in the existing SQLite index, preserving literal link matching, source order and formatted output. Use indexed note summaries and orphan detection for status queries.

Refresh affected entries after writes, renames and project archival. Detect external edits with a 15-second metadata census in both transports. Isolate maintenance failures, avoid repeating another process's refresh, and preserve complete shared rows during rebuilds and concurrent census removals.

Upgrade: run the new executable with --migrate-index before the first v0.3.1 server start, using exactly the configured FOLIO_ROOT path. Follow the README upgrade procedure. External edits can take up to 15 seconds to appear; writes through this version refresh affected entries before returning.
